GUIDELINES FOR STUDENTS AND FACULTY
 

Faculty Guidelines

  1. Instructors should deliver in person or via a sealed envelope all copies of their test(s).
  2. Instructors must complete a Referral Form specifying number of tests sent to the center, test guidelines, and listing any materials, equipment, or special services.
  3. Instructors / students must furnish scantrons when necessary.
  4. Instructors must make special arrangements for any (taken or untaken) tests which are not picked up by semester's end; otherwise the Assessment Center will return all tests to the instructor's boxes at this time.
  5. The Assessment Center administers, mails, and handles only tests taken. The Assessment Center cannot accept handouts or syllabi from instructors, special projects from students, homework assignments from students to instructors, or mail non-test materials.
  6. An Incident Form will be used to report problems or special occurrences.
  7. The Center will not administer tests without a Referral Form.
  8. No faxed tests will be accepted.
  9. No testing can begin 30 minutes prior to closing.

Student Guidelines

  1. All students must show a photo ID.
  2. All students must sign-in.
  3. No testing can begin 30 minutes prior to closing.
  4. Student should know their instructor's name. Tests are filed by subject and instructor.
  5. Students will be asked to leave all books and backpacks in a designated space. The assesment center is not responsible for items left in this area.
  6. Students will be asked to turn off phones and pagers.
  7. Students are not allowed to leave the Center unless it is an emergency, and they must consult the proctoring staff before leaving.
  8. No breaks allowed.
  9. No food or drinks allowed.
  10. Students must furnish their own supplies (i.e. pencils, pens, scantrons, etc.).
  11. Talking is not permitted in the Assessment Center.
  12. An Incident Form will be used to report problems or unusual occurrences.
  13. If a student is suspected of cheating, the student will be closely observed. The proctor will document the incident on the Incident Form and a copy will be given to the instructor.
  14. When students are finished, the tests are returned to the proctor. No homework or outside materials accepted. Students are not allowed to remove tests from the Assessment Center.
