HOW TO ACCESS THE SERVICES PROVIDED

Register with the Disability Services Office (mandatory)

Make an appointment with the DSO staff

Bring any documentation of disability (i.e. records of past service from a DS provider, doctor's evaluation, TRC or TCB referral)

If no documenation is available they will be referred to the appropriate resources to be tested and evaluated.

The Disability Services Coordinator or counselor will fill out a Request for Services form. The student willshow the form to the professor and/or the AT staff when a request for service or modification is made. The form will notify us of the services needed by the student that have been documented and approved by the DSO.

If the student requires alternatively formatted handouts or tests the student must show the checklist to the professor and request that the tests be provided to the AT lab manager in enough time to allow for its production. We prefer that all handouts and tests be provided to us at the beginning of the semester so that we can get them ready in plenty of time. The professor must provide a completed Assessment Center form along with the test if it is to be sent to the Assessment Center. If the professor prefers to have the documents printed as they are needed we will need at least 48 working hours notice. Some tests, especially technical material, will take longer to complete so please ask if more time will be needed for a particular test or not. Please only leave tests with the AT lab manager for test security reasons.
