Guidelines for Students and Faculty
The Assessment Center offers an alternative classroom for faculty, distance learning, and makeup testing. A test referral form is required prior to testing.

Faculty Guidelines
- All tests should be delivered in person or by interoffice mail, no faxed test will be accepted.
- Instructors must complete a Referral Form specifying the students permitted to take the test, the number of tests sent to the center, test guidelines, materials allowed, and special services.
- The assessment center will not administer any test without a completed referral form, class roster on file or the name of the students permitted to take the test listed on the referral form.
- Instructors must indicate on the referral form what they would like us to do with the completed test (interoffice mail to the office or hold in the center for pickup). The week of finals all test will be held in the Assessment Center for pick up.
- Instructors must make special arrangements for any (taken or untaken) test which are not picked up by the end of the semester; otherwise the Assessment Center will return all test to the instructor’s mail box via interoffice mail.
- The Assessment Center does not accept or issue any non-testing materials such as syllabi, projects, take-home test, or homework.
- There will be no entire class testing in the Assessment center for any classes that are not flex-study or distance learning.
- There will be no “anyone requesting ”accepted.
- The Assessment Center will provide the instructor with an incident form stating any problems or special concerns.
Student Guidelines - All students must sign in and show a photo ID.
- No testing can begin 1 hour prior to closing.
- All pagers and cell phones must be turned off and left with the proctors along with a photo ID.
- Students must leave all books and backpacks in their vehicle or designated area.
- No food, drink, or gum allowed.
- No children please.
- Students must know the name of their instructor, class, and test.
- Students are responsible for bringing scantrons and any other materials required for testing.
- Students are not allowed to leave the Assessment Center unless it is an emergency. The proctor must be consulted before the student leaves the Assessment Center.
- Talking is not permitted inside of the testing rooms.
- Students will be closely monitored. If the student is suspected of cheating, the proctor will inform, but not confront the student during testing. The proctor will document the on the Incident Form and a copy will be given to the instructor.
- When students have completed their test, they must return it to the proctor. Students are not allowed to take test out of the testing center.
